By continuing to sign up, you agree to the following terms:

I hereby waive, release, and discharge any and all claims for damages for personal injury, death, property damages or theft or which may hereafter occur to me as a result of participation in any activities that are arranged or organised by, or on behalf of Masoomin Sports Club. These activities include, but not limited to sports sessions/sports training and social gatherings.

It is understood that some of these activities involve an element of risk or danger of accidents to myself, and knowing those risks, I hereby assume those risks.

I understand that at any of these activities, my child may be photographed. I agree to allow my child's photo, video, or film likeness to be used for any legitimate purpose by the event holders sponsors and/or organisers.

I understand that it is my responsibility to have the appropriate clothing, protection attire and equipment suitable for the particular event, and absolve Masoomin Sports Club of and discharge any and all claims for damages for personal injury, death, property damages or theft or which may hereafter occur to me as a result of inadequate clothing, protection, attire and equipment.

I understand that it is my responsibility to adhere to the rules and regulations set out by each venue/location where an event is held and absolve Masoomin Sports Club of and discharge any and all claims for damages for personal injury, death, property damages or theft or which may hereafter occur should those rules and regulations be ignored.

Masoomin Sports Club reserves the right to modify a sports programme and/or cancel due to low registration numbers, adverse weather conditions or other unforeseen circumstances.

Consent for Treatment

I hereby consent to receive medical treatment that may be deemed advisable in the event of injury, accident, and/or illness during any Masoomin Sports Club events. It is understood that Masoomin Sports Club will provide no medical insurance for such treatment, and that the cost thereof will be at my expense.
